Adding rate code to the clan website page

Please no flaming, just trying to get something done. Alright, I just can't fiqure out how to get the rate code into my website for the poll to come up. I have taken out the URLHERE to my site's url and still hasn't worked. Could anyone help me with this or tell me how?
